Policy and Procedures FormStudent's Name* First Last Lesson Policy and ProceduresPlease check each box after reading the items below.Tuition* Tuition due the 1st of the month.Payment is made through Auto-Pay.Absences* Please notify our office of absences by texting our attendance line: 303-439-795624 Hours Notice is Required for Make-up Lessons, otherwise the lesson is forfeited. Our office handles the scheduling not the instructors. Missed lessons cannot be refunded.Make-Up Lessons* Make-up lessons will be given for lessons cancelled with 24 hours notice- Make-up lessons last for 6 months after original lesson - We can only schedule make-up lessons or pre-bookings up to 2 weeks out. - Make-ups can not be applied to tuition payments. - Any make-up lesson scheduled and then canceled cannot be made up againStopping Lessons* 2 Weeks’ Notice is Required to stop lessonsLessons are a month-to-month commitment even though your spot is held on the schedule indefinitely. Middle of the month is the best time to let us know you’ll be stopping lessons. Please give us the chance to correct any concerns you may have before stopping lessons.Materials* Students will be given a folder and backpack for their music.Please bring it to every lesson to prevent the instructor from wasting lesson time on reprinting.Possible Instructor Changes* We do our best to keep our instructors with us long term.If we should lose an instructor, students will be placed with another instructor that is the best fit for them.Substitute Instructors* The student might have a substitute instructor.If your instructor is unable to teach a lesson, a qualified substitute teacher will teach the lesson. If no substitutes are available, the lesson will be rescheduled. No refunds or credits will be given, only make-up lessons.Inclement Weather* No credits or refunds will be given due to weather.Make-ups will be scheduled. Please check the news. We will cancel lessons if Boulder Valley or Adams 12 Schools cancel.Holidays* Lessons are year-round.Make-ups will be scheduled if there are less than 4 lessons in that month. Holidays we are closed: Memorial Day, Labor Day, 4th of July, Thanksgiving Day, Black Friday, Christmas Eve & Christmas Day, New Years Eve & New Years Day. No refunds or credits will be given.Communication* Keeping communication open to better serve you and the studentInstructors will give you a quick progress report after each lesson. Please let us know if any concerns arise or if your child starts to lose interest in lessons or is not practicing at home. We have different methods we can try.Multiple Instruments* Our instructors teach multiple instruments.If a student is losing interest in their lessons, we suggest trying another instrument. Instructor will discuss this with you before switching instruments.Student Photo* Scheduling photo on fileOur scheduling software uses photos to help us identify our students.Promotional Photos* We take photos during all performances and occasionally during lessons.Photos of your child might be used to feature your child in the studio or on our Facebook page.Payment Authorization* I authorize Globalsound Studio to automatically charge my account on the 1st of every month to the credit card or bank information that I have provided.I may also give verbal permission to an office staff member to charge my account for other transactions such as the Rock Band Program or needed equipment. I will be emailed a receipt when my account gets charged.We thank you in advance for your cooperation on these procedures. Good instructors are hard to find.
